چکیده

PURPOSE Inguinal hernias often occur after radical retropubic prostatectomy (RRP). We present a novel and simple technique for preventing inguinal hernias after RRP, which any surgeon can complete within a few minutes. METHODS A total of 230 Japanese prostate cancer patients underwent RRP between January 2007 and September 2011. From July 2009, 115 patients underwent inguinal hernia prevention procedures at the same time as RRP. In this procedure, we released approximately 5 cm of the bilateral vas deferens and spermatic vessels from the peritoneum. In cases in which the processus vaginalis had spread into the abdomen, we ligated it close to the peritoneal cavity and then transected it. The remaining 115 patients who underwent RRP but did not undergo the hernia prevention procedure were used as the control group. The incidence rate of postoperative inguinal hernia was compared between the 2 groups. RESULTS Inguinal hernias developed during the postoperative follow-up period in 18 of the 115 control patients (15.7%) (median duration, 50 months). The hernia-free survival rate of this group was 89.6% and 84.1% at 1 and 2 postoperative years, respectively. In contrast, only 1 of the 115 patients (0.87%) who underwent the hernia prevention procedure developed an inguinal hernia during the follow-up period (median duration, 27 months). The hernia-free survival rate of this group was 100% at both 1 and 2 postoperative years (P<0.0001). CONCLUSIONS We developed a simple method for preventing post-RRP inguinal hernias. The procedure is easy to perform and produces excellent outcomes.

PURPOSE In this retrospective study, we aimed to compare the clinical characteristics of inguinal hernia developed after radical retropubic surgery for prostate cancer to the hernia without previous radical prostatectomy. Laparoscopic Radical Prostatectomy Following Laparoscopic Bilateral Mesh Hernia Repair

Laparoscopic mesh hernia repair is an effective form of management of inguinal hernias. Polypropylene mesh is generally placed at the internal rings extending across the midline resulting in an intense fibrotic reaction that can make subsequent radical retropubic prostatectomy and lymphadenectomy difficult. We report the first case of laparoscopic radical prostatectomy following laparoscopic bi...

Urinary straining contributes to inguinal hernia after radical retropubic prostatectomy.

OBJECTIVES To verify whether abdominal pressure during urination represents an important factor in the postoperative development of inguinal hernia after radical retropubic prostatectomy. METHODS Participants comprised 228 patients who underwent radical retropubic prostatectomy without prophylaxis for inguinal herniation between 2002 and 2007. Development of inguinal hernia was assessed from ...

Sexual rehabilitation after radical retropubic prostatectomy: new technique using ilio-inguinal nerve graft.

Sural nerve grafts have been used to repair the cavernous nerves and allow sexual potency recovery after radical prostatectomy. In order to overcome the drawbacks related to the harvesting of the sural nerve, the authors developed an original technique using ilio-inguinal nerve grafts to repair the cavernous innervation during radical prostatectomy. This technique used in a group of new patient...

Incidence of inguinal hernia after radical retropubic, perineal and laparoscopic prostatectomy.

We would like add our experience of inguinal hernia (IH) after radical prostatectomy (RP). Sekita et al. found a high incidence of IH of 23.9% after prostate surgery. I commend them on a study with a good comparison group of those patients having a transurethral resection of prostate (TURP). We also looked at IH rates after RP in a retrospective study of 579 patients having radical retropubic p...
